| Details | Continuum of Care enables people who are challenged with mental illness, intellectual disabilities (including Autism) and/or struggling with addiction to rebuild a meaningful life and thrive in the community. Continuum provides housing, therapy, case management, support and hope to some of the most physically and emotionally vulnerable people in our community. Continuum has a groundbreaking crisis intervention program called COMPASS (Compass Allies Serving our Streets) that enables mental health and substance abuse experts to respond to 911 calls alongside police and first responders. The COMPASS Team responds when a situation involves individuals with mental health or substance abuse who are in a state of crisis. It is made up of social workers and peer recovery specialists who immerse themselves into the community to find people in need and connect them to the right care. The COMPASS Team has had significant impact on the individuals they encounter. Many of them are homeless and have tremendous need. We help them stabilize, get access to basic needs (food, clothing), and also get connected to the treatments and therapies they need to begin to turn their lives around. Already the team has helped hundreds of individuals so far. Won’t you help too?
